You'll be assisting vets with obtaining housing and sustaining housing.
_*You'll be given training so experience with VASH is not necessary nor experience working with the homeless population.

Note: The office is based in Santa Rosa however, you'll be working in the Ukiah area.
*This is field based work however you will have an office and can also work from home however, the expectation is to be in the community.

Note: Clinical staff must be willing to commit to at least one year.
*Professional Experience*
* Experience providing direct mental health and/or intensive case management services to vulnerable populations
*Responsibilities*
* Assess Veterans through comprehensive psychosocial evaluations to determine case-management needs, recovery goals and provide referrals to appropriate treatment or services.
* Work with the Veterans to obtain housing voucher from the PHA, provide housing search assistance, facilitate smooth move-in and ongoing case management to ensure housing retention.
* Employ Motivational Interviewing and Harm Reduction approaches to promote Veteran follow-through with referrals for: preventive care and treatment of medical conditions, substance use and dependence, other mental health conditions, and problematic health behaviors.
* Conduct regularly scheduled home visits to assess Veteran’s housing stability, social connection, recovery, and ensure residence is safe and in compliance with local housing codes.
* Act as a liaison with VA and community resources, landlords, and PHA.
* Assist Veterans to identify housing goals, develop skills that increase self-sufficiency and eliminate barriers.
* Lead housing retention group for Veterans
* Collect and submit HUD-VASH program evaluation data on Veteran participants.
* Identify opportunities to collaborative with local apartment complexes, rental agencies, and community services.
* Preform quality assurance checks on a quarterly basis.
* Attend agency meetings and participate in community partner meetings
Job Type: Full-time
Pay: $72,000.
