POSITION DESCRIPTION *Position:* Behavioral Health/Substance Use Navigator *Reports to:* Director of Behavioral Health *Worksite:* Sutter Lakeside Hospital Emergency Dept.
*Classification: *Full-Time 32 hours per week SUMMARY: The Navigator shall provide substance use screenings, advocate for follow-up individual/group treatment, therapy, discharge and aftercare planning to patients and their families.
The Navigator is one of the primary contacts for involving and educating patients, families, and significant others in the process of substance use and mental health treatment and recovery.
They shall also serve as a liaison to community programs and develop strategic partnerships to enhance aftercare plans of patients and their families.
The Navigator assists patients with substance use disorder (SUD) by supporting treatment in the hospital (buprenorphine, methadone, and naltrexone), help patients with mental health disorders by completing brief assessments and crisis interventions, coordinating with tele-psych assessment and with Lake County Behavioral Health regarding hold status and placement, and by connecting patients to follow-up care including MAT (medication addiction treatment), and outpatient mental health care.
Navigators do this while simultaneously advocating for culture change to reduce stigma and discrimination.
*ESSENTIAL FUNCTIONS:* The Navigator shall participate on committees as required and will provide care coordination as an integral team member of a statewide initiative to improve access to Medication for Opioid Use Disorder (MOUD).
The Navigator shall work with patients in the Emergency Department, inpatient, and outpatient settings for those who are interested in MOUD.
They shall conduct initial brief assessments, introduce patients to MOUD programs and services, expedite appointments at MOUD-capable clinics, serve as the primary coach for their clients, and maintain ongoing contact with their panel.
They shall troubleshoot pharmacy and insurance issues.
They may also assist with referrals (and expedite appointments if possible) to other services such as primary care, mental health services, social services, and residential treatment facilities.
* Support feasible and effective treatment of substance use and mental health disorders in busy, overcrowded hospital emergency departments.
* Assist with identification of patients with a substance use disorder in the emergency department and, where feasible, within inpatient units, by monitoring patient tracking systems to screen for eligible patients and checking in with clinicians and nursing staff to receive referrals of eligible patients.
* Assist patients with SUD get into treatment for their addiction so they are less likely to require a repeat visit to treat infections or other serious conditions.
* Provide services in a nonjudgmental, energetic, positive, harm-reduction approach to assist patients in the emergency room with substance use disorder (SUD).
* Work with Lake County Behavioral Health to assist patients with MH disorders get into treatment when they are on an involuntary hold (psychiatric hospitalization) and help connect them with outpatient behavioral health services, so patients are less likely to require a repeat visit.
* Approach people who use substances or alcohol, or experience MH symptoms, with respect and compassion so that patients view the hospital as a safe place to seek care for SUD, AUD and MH disorders and other emergency medical conditions.
* Understand Substance Use Disorder as a medical condition and the role of MAT in treatment and harm reduction.
Adherence to an abstinence-based social model that is unsupportive of MAT is not aligned with the goals of this position.
* Acts as patient advocate in negotiating the health care delivery system and in accessing community resources.
* Assist patients and families in problem solving and decision-making regarding discharge planning options, addressing physical, social, emotional, and financial concerns.
* Facilitate interdisciplinary team communication and patient care management through coordination of team conferences, patient care conferences and family conferences as needed.
* Complete all medical record documentation in accordance with LFRC and SLH standards.
* Comply with all required standards for medical/legal situations, including phone reports and written documentation for all suspected abuse.
* Attends meetings as required.
* Develop and record, in EPIC, treatment plans and goals based on psychosocial assessments, in accordance with departmental standards.
* Refer and assist patients/families in applying for appropriate financial programs (CCS, SDI, SSI, SSD, private pensions) and legal instruments as needed.
* Assist in implementing plan of care by making timely referrals for community-based services as indicated.
* Assists patients in applying for Medi-Cal, if eligible and appropriate, for continued medical insurance coverage.
Educates patients/families in the realities and limitations of health care coverage.
* The Navigator may provide other services as required or assigned by the Customer.

POSITION SPECIFICATIONS: Licensed Clinical Social Worker (LCSW) licensed to provide services in the State of California.
* Master’s degree in social work from an accredited CSWE.
Required 3 years of practical experience in underserved communities, preferably in social services, mental health, substance abuse or related fields.
* Bilingual (English/Spanish or ASL) a plus.
* Knowledge of medical terminology a plus.
* Diplomacy in resolving conflicts and integrating differing perspectives.
* Ability to handle escalated situations safely and appropriately.
* Ability to represent core agency/program values and principles in a variety of settings.
* Demonstrate effective critical reasoning and problem-solving skills.
* Model effective self-management and coping techniques.
* Willingness to work flexible schedules as deemed necessary including weekends, and evenings.
* Familiarity with Zoom platform, and other virtual programs.
* Ability to work in a team environment, while also working independently.
* Demonstrated capacity to handle multiple tasks, projects and meet deadlines.
* Good verbal and written communication skills.
* Good public relations, conflict management and meeting facilitation skills.
* Must maintain confidentiality with sensitive information.
* Proficiency in data entry, Microsoft Office 365 Suite especially Word, PowerPoint, and Excel.
* Valid CA driver’s license and insurance.
*SKILLS AND KNOWLEDGE:* * Ability to evaluate patient and family needs and to effectively plan and implement social work interventions in a health care setting.
* Ability to communicate effectively orally and in writing.
* Ability to work as member of a team.
* Ability to work in a very dynamic setting and to set priorities.
* Knowledge of substance use disorder as a medical condition and the role of MOUD in treatment and harm reduction.
