Responsibilities:
- Provide wound care and treatment to patients with acute or chronic wounds
- Assess and monitor wounds for signs of infection or complications
- Develop and implement individualized care plans for patients with complex wound care needs
- Collaborate with interdisciplinary healthcare team members to coordinate patient care
- Educate patients and their families on wound care techniques and prevention strategies
- Document patient assessments, interventions, and outcomes accurately and timely
Qualifications:
- Valid nursing license and wound care license in the state of practice
- Minimum of [1] years of experience as a Wound Care Nurse
- Proficiency in ICD-9 coding for accurate documentation
- Knowledge of hospice palliative medicine principles and practices
- Ability to catheterize patients as needed for wound care procedures
- Competency in taking vital signs and monitoring patient status
- Experience working with individuals with developmental disabilities is a plus
- Familiarity with family planning and reproductive health services
- Understanding of enteral nutrition principles, including tube feeding administration
- Knowledge of laboratory tests related to wound care management
